Submission of a “Letter of Inquiry”

The Foundation requires that a one to two-page “letter of inquiry” be submitted to initiate a request for future funding consideration. The letter should include the following:

  • Mission of the organization
  • Population served
  • Scope of program/activities
  • Key accomplishments/impact statements
  • Summary of project including goals and objectives
  • Plan for tracking and evaluation including measurements that will be used to determine impact, success and sustainability
  • Amount of funding requested
  • Grant period

Applicant will be contacted to provide a full proposal if the Board of Trustees is interested in obtaining additional information for further consideration. You will be provided a password to access the online application or to download the application form.
